Patient Referrals

Recovery Keys welcomes referrals from medical professionals in Jacksonville and St. Augustine  for physician-led, Joint Commission accredited treatment for patients in need of treatment for addiction to alcohol, drugs, opioids and/or pain medications.

After completing our confidential and secure patient referral form, a representative from Recovery Keys will reach out to request transfer of the patient’s medical records and confirm details of their situation. We will then contact the patient to follow-up and schedule a comprehensive addiction evaluation, diagnosis and treatment.
